SERIOUS INCIDENT & FATALITY INVESTIGATIONS

Support through
the hardest days.

When a serious incident happens, the next steps can feel overwhelming. You don’t have to navigate them alone.

We help employers find the facts, work through regulatory demands and support the people affected—with a clear path forward.

Practical experience navigating occupational fatalities.

CLARITY. CARE. A WAY FORWARD.
WHEN THE STAKES ARE HIGHEST

Your people.
The facts.
The next step.

EXPERIENCED SUPPORT. HUMAN UNDERSTANDING.

One incident.
So much to navigate.

Questions from OH&S. Documents to gather. Workers who are hurting. Decisions that can’t wait.

100% Safety helps you bring structure to the response, so your organization can take the next step with greater clarity.

01

Understand
what happened.

A thorough investigation goes beyond the immediate event to examine the decisions, conditions and systems behind it.

  • Evidence and document review
  • Witness accounts and timelines
  • Causal factors and corrective actions
  • Internal investigation reports
02

Work through
the requirements.

Regulatory demands and orders bring their own pressure. We help organize the work and prepare clear, factual responses.

  • OH&S demand-response preparation
  • Order compliance support
  • WCB fatality reporting assistance
  • Coordination with OH&S legal counsel
03

Look after
your people.

The impact reaches well beyond the scene. Workers, supervisors and your own safety team may all need support.

  • Crisis counselling coordination
  • Practical support for internal teams
  • Factual employee communications
  • Respect for families and privacy

For employers, prime contractors, business owners and internal safety teams facing a serious injury, reportable incident or workplace fatality.

A CLEARER PATH THROUGH

From the first call
to the follow-through.

Every incident is different. We shape our support around what has happened, where you are in the process and what your team needs next.

01 / RESPONSE

Bring order to
the first decisions.

Once emergency response is underway, the organization still has important responsibilities. We help identify priorities and coordinate the next steps.

Where we can help

  • Identify reporting and documentation priorities
  • Plan evidence preservation around scene restrictions
  • Coordinate OH&S counsel and crisis support
  • Clarify responsibilities within your response team
THE FOCUS

Clear priorities. Defined responsibilities. Support for the people affected.

Our role is practical HSE and investigation support. Legal advice and representation come from your OH&S lawyer.

Care belongs in the response.

THE HUMAN SIDE OF THE WORK

There are people
behind every
investigation.

A serious incident can affect an entire workplace. The people expected to lead the response may be grieving, in shock or carrying the weight of what happened.

We bring practical support alongside your team, help connect affected people with crisis counselling, and keep dignity and respect at the centre of the process.

Your safety team needs support, too.

Even experienced internal safety professionals may never have navigated a workplace fatality. They shouldn’t have to carry the response alone.

QUESTIONS YOU MAY HAVE

A little clarity
helps.

You don’t need to have every answer before reaching out.

Let’s talk about your situation
We already have a safety person. Can you still help?

Yes. We can work alongside your internal safety team, take on specific parts of the investigation or help coordinate the wider response. A workplace fatality can place significant demands on the people you normally rely on.

Does the OH&S investigation replace our own?

No. For incidents covered by Alberta’s reporting requirements, the prime contractor—or the employer if there is no prime contractor—has its own investigation and reporting responsibilities. We can help with that work.

Read Alberta’s investigation requirements ↗
What if no one was injured?

An incident may still be reportable. Alberta’s potentially serious incident requirements consider the likelihood of serious harm and whether corrective action may be needed. The reporting process differs from serious-incident notification.

Do we need an OH&S lawyer as well?

We strongly encourage employers navigating a fatality or major reportable incident to retain OH&S legal counsel. We provide HSE and investigation support, and can help coordinate with counsel. We do not provide legal advice or legal representation.

Can you help with orders and corrective actions?

Yes. We can help organize the compliance work, update hazard assessments or safety programs, coordinate specialist input and prepare documentation. The issuing authority determines whether an order has been satisfied or can be lifted.
